No matter where you live in Minnesota, UCare has a Medicare plan to fit you and your wallet. You can choose the level of coverage that’s right for you and decide if you’d rather:

• Pay a higher premium and pay less for care when you need it
• Pay a lower premium and pay more for care when you need it

Some of the benefits you may enjoy in 2024:

• Coverage when traveling — Coverage varies by plan. With UCare Anywhere℠, you can travel in the U.S. and get care from any provider that accepts Medicare, and you have emergency coverage worldwide. Many of our plans also include in-network coverage within the U.S. when you see providers who participate in the MultiPlan Network.
• One Pass fitness benefits — a basic membership at more than 24,000 participating fitness centers at no cost to you.
• Community education discount – Get up to $15 off most Minnesota community education classes, like cooking or learning a new language. Check your local community education catalog or contact the local school district for class times and locations. Limit of three discounts in a calendar year (one discount per class enrollment). Not available with UCare Your Choice plans.
• $0 copay for mental health office visits

Am I eligible for coverage?

To become a UCare Medicare member, you must:

• Have Medicare Part A and Part B (by age or disability)
• Live in the UCare Medicare service area - which includes the entire state of Minnesota. Note: Some plans are not available in all counties.

What plans are available to me?

Health care costs can vary depending on the area in which you live. Our coverage area is divided into three regions based on counties: North, South and Metro. Find your county on the list below or view the map of UCare Medicare regions to find out which region you are in.

Can I get dental coverage too?

All our Medicare plans include access to dental care. The level of coverage varies by plan.

• UCare Aware, UCare Your Choice, UCare Your Choice Plus, EssentiaCare Access, and Care Wise have a dental allowance.
• UCare Complete, Value and Care Core include routine and restorative dental for no extra premium.
• UCare Essentials Rx, Standard, Classic, Value Plus, EssentiaCare Secure and Grand have routine dental with optional coverage available

You will get the best benefit by using a network dentist. UCare Medicare Plans use the Delta Dental Medicare Advantage Network administered by Delta Dental of Minnesota. UCare Your Choice plans and EssentiaCare Access do not require use of a dental network.
